When transitions are done well, they are about much more than moving from one home or service to another. They are about listening carefully, following the right processes, bringing the right people together, and making sure each person feels safe, informed and supported every step of the way.
Lizzy’s story is a great example of teams working together The Right Way to manage a transition with care, consistency and respect.
Lizzy’s story
Lizzy had previously lived with Aruma in Port Macquarie, before moving to Casino to live with her sister in in October 2024. When the opportunity arose for Lizzy to return to Port Macquarie, her support network looked to Aruma again.
Manager Shared Living, Blake Turpie helped coordinate a video call so Lizzy could reconnect with her old housemates, Linda, Sarah and the team. The call gave Lizzy the opportunity to see the environment, talk through the move and feel included in what was happening. She was excited about returning, and the team could see how much the connection still meant to her.
Transition planning takes careful coordination across Operations and Shared Services teams, including Clinical, Quality, Safeguarding and Practice, and Business Development. For Lizzy, the team took the time to involve the right people early, so the move could be planned safely and sustainably.
Because the transition was managed thoughtfully, Lizzy’s return felt familiar and positive. Team members described it as though she had never left. The consistent relationships, careful preparation and person-centred approach helped Lizzy settle back into her home and community.
Regional Manager, Peter Jenkins reflected, “This is why we’re here. Lizzy’s resilience, together with the team’s commitment to doing things The Right Way made the transition a positive experience. We’re all glad to have Lizzie back.”
